Medication Safety High-Alert Drugs LASA Error Reporting Smart Pumps

About the ISMP Summit

The ISMP Medication Safety Summit ↗ is the flagship conference of the Institute for Safe Medication Practices ↗ — the independent US non-profit that analyses medication error reports, issues safety alerts, and develops evidence-based prevention guidelines used by hospitals, pharmacies, and regulators nationwide.

The 2026 Summit in Philadelphia addresses the most pressing patient safety challenges facing health systems today: smart infusion pump programming errors, high-alert medication failures, look-alike/sound-alike (LASA) name confusion, and the transition to electronic prescribing systems with inadequate safety checks. See the medication errors guide for the NCC MERP error taxonomy used throughout ISMP's work.

ISMP high-alert medications — selected examples

Drug / classError typeKey safeguard
Concentrated KCl (≥2 mEq/mL) IV bolus instead of diluted infusion → cardiac arrest Remove from floor stock; pharmacy-only dispensing
Insulin (all formulations) Unit/mL confusion; U-500 for U-100; wrong pen Standard concentrations; insulin-only syringes; double-check
Unfractionated heparin (IV) 10-fold overdose; wrong weight-based protocol Pre-printed weight-based protocols; independent check
Warfarin Missed INR; drug interactions not acted on INR monitoring alerts; DDI screening at dispensing
Neuromuscular blocking agents Given instead of sedative; patient not ventilated Separate storage; warning label "causes respiratory arrest"
IV opioids (morphine, hydromorphone) 10× overdose; hydromorphone confused with morphine Smart pump drug libraries; tall-man lettering; PCA limits
Methotrexate (oral low-dose) Daily instead of weekly → fatal "Weekly dose only" labelling; dispensing alerts; patient education

Frequently asked questions

What is the ISMP Medication Safety Summit?
The ISMP Medication Safety Summit ↗ is organised by the Institute for Safe Medication Practices (ISMP) ↗, the US authority on preventing medication errors. It unites pharmacists, nurses, physicians, and technicians to address systemic error causes via case studies and prevention strategies. Background reading: medication errors classification guide.
What is ISMP's role in medication safety in the US?
What are high-alert medications and why do they matter?
High-alert medications bear heightened harm risk when used in error. ISMP's list includes: insulin ↗, anticoagulants (warfarin ↗, heparin, DOACs), concentrated electrolytes (KCl ≥2 mEq/mL), opioids, and neuromuscular blocking agents. Safeguards include independent double-checks, concentration standardisation, and tall-man lettering. See medication errors guide for the full NCC MERP classification.
What is the ISMP LASA drug list and how is it used?
ISMP's List of Confused Drug Names ↗ identifies look-alike/sound-alike pairs. Implementations include: tall-man lettering (e.g., hydrALAzine vs hydrOXYzine), physical separation in dispensing systems, alert flags in pharmacy IT systems ↗, and staff education. Medication errors guide covers LASA classification in detail.
What is the ISMP National Medication Errors Reporting Program (MERP)?
MERP ↗ accepts confidential voluntary reports from healthcare practitioners. ISMP analyses patterns and issues safety bulletins — many have driven FDA label changes and product redesigns (e.g., elimination of concentrated KCl from floor stock after multiple fatalities). For clinical context, see the medication errors classification guide and the FDA medication error reporting framework ↗.
